Salary: $48,000-$53,000/year (based on experience) + benefits
Description
Dual role of Coordinator of Disability Services whom is responsible for ensuring equal and timely access for all individuals seeking Mental Health and/or Disability Service within the region. While responsible for the efficient, accurate and timely performance of a variety of Support Staff, clerical, receptionist and secretarial duties for Social Services Office.
Duties
- Assist individuals in completion of the Application and collection of the supporting data
- Serve as liaison between Chief Executive Officer and the individual
- Complete Standardized Functional Assessment for all individuals seeking services within 90 days of receipt of a signed Application.
- Make referrals and coordinate services between all service providers.
- Completion of Re-certification on an annual basis
- Participate in multi-disciplinary team meetings.
- Seek assistance from a professional who is trained in the delivery of the mental health or disability service for decisions that are clinical in nature.
- Assist in processing placements from institutions and coordinate placement requests from Case Management.
- Participate in Regional Planning processes including attendance at various boards and advisory meetings.
- Assist in the development of the Regional Management Plan.
- Provide county data and expenditure information to the Chief Executive Officer as requested.
- Serve as a liaison to the court system to assist with the committal process as needed.
- Assist in the development of long-range plans for the delivery of services within the region.
- Attend trainings and conferences to develop and maintain knowledge of Iowa Administrative Code and changes occurring within the Human Service Field.
- Supervises agency staff as applicable and directed by the Chief Executive Officer.
- Maintains valid driver's license and proof of insurance at all times and willing to travel between counties.
- Processing and determining eligibility for MHDS applications completed by or for those individuals residing or having residence in Woodbury County.
- Completing a Funding Request in the Community Services Network (CSN) with recommendation to the CEO.
- Maintaining the client's electronic file in CSN
- Following-up and assisting clients regarding their eligibility
- Assist with walk-in traffic, answer phones, sort and distribute mail and facsimiles as needed.
- Provide administrative support as directed by the Rolling Hills CEO.
- Communicate with Clerk of Courts office regarding involuntary hospitalization court committal claims and information.
- Assist Woodbury County Sheriff's office with information relating to gun permit applications.
Required Qualifications
Experience and Education
- Bachelor's or higher-level degree in human services-related or administrative-related filed, including but not limited to social work, psychology, nursing, or public or business administration, from and accredited college or university.
- In lieu of a degree in public or business administration, a coordinator may provide documentation of relevant management experience.
- A minimum of two years' experience in the provision of services in human serves-related field.
