Clinical Case Manager – Nursing - Case Management
Upstate Medical University is looking for a Registered Nurse to join their dynamic team of expert professionals.
The Clinical Case Manager will facilitate the continuum of care for an assigned number of cases by managing, coordinating, facilitating and monitoring resources to achieve optimal clinical outcomes, utilization of services and financial goals. The Clinical Case Manager oversees the discharge process for all patients within the service area and is an integral team member in efforts to establish a safe and appropriate discharge plan/plan of care.

NYS licensed RN (or eligible), Bachelor's degree in Nursing or healthcare related field, plus minimum of three (3) years acute care RN or acute care, transitional care, or chronic care RN/case management experience required. CPR-BLS certification required by time of appointment. Master's degree in Nursing or healthcare related field and prior acute care case management experience preferred; PRI certification a plus.
