Case Manager (2) Mental Health Support Services
Hourly Range: $21.49 - $25.30

Chesterfield County's Mental Health Support Services Department is seeking two part-time Case Managers to provide services to adults with intellectual disabilities. Functions include assessing consumers' strengths and needs; developing individualized service plans; completing written service assessments; linking consumers with other community resources; monitoring sufficiency of services and goal attainment; collaboration with other professionals. Perform other work as required.

Successful candidates will possess a bachelor's degree in a human service field with two years of specialized coursework in case management skills and working with people with disabilities; or an equivalent combination of training and experience. Must be knowledgeable in progressive case management, state and federal regulations for community and ICF services, and possess computer skills necessary to enter data into an electronic medical record. Must meet the criteria for a Qualified Intellectual Disability Professional (QIDP). Good driving record required. Must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months. Pre-employment drug testing, FBI criminal background check and education/degree verification required. A Chesterfield County application is required and must be submitted online by deadline.
